Reevaluation of Male Partner with Normal Findings in Conventional Semen Analysis by Sperm Morphology Evaluation Using Strict Criteria



Abstract
This study was performed to reveal the incidence of male factor infertility diagnosed by sperm morphology evaluation using strict criteria(SMEUSC) in male with normal findings in conventional semen analysis and to assess the effectiveness of
SMEUSC
as a
routine screening test in infertile couples, from June to September 1994. Fifty-five infertile couples who had undergone intrauterine insemination(IUI) during ovarian stimulation cycles for the treatment of infertility due to ovulatory
factor(n=40)
and
unexplained infertility(=15) were allocated to the study group. Ovarian stimulation was performed using clomiphene citrate(n=34), human menopausal gonadotropin(hMG) (n=5), and ultrashort protocol of gonadotropin-releasing hormone(GnRH)
agonist(n=16).
All male partners included in this study had normal findings in conventional semen analyses. Of the 55 patients studied, 7(12.7%) had a normal sperm morphology below 2.5% and 8(14.5%) had a morphology index(summation of normal and slightly
amorphous
morphology) below 5.5% in SMEUSC. All patients with <2.5% normal sperm morphology had a morphology index below 5.5. All patients with <2.5% normal sperm morphology did not become pregnant. All patients with <5.5% morphology index except a
patients
did
not become pregnant. A patients with 5.3% morphology index became pregnant through IUI during ovarian stimulation cycle using ultrashort protocol of GnRH agonist.
This study demonstrates that SMEUSC is simple, inexpensive and useful and it could be required as a routine screening test for the appropriate therapeutic approach in infertile couples.
